Become A Patient
Salvus Center, Inc., operates like a primary care doctor’s office. We offer examinations, imaging procedures, and laboratory tests. The staff includes two (2) Supervising Physicians, two (2) full-time nurse practitioners, two (2) LPNs and one (1) MA/Interpreter. We also work with specialty physicians who provide services to our patients in the clinic and in their offices. The clinic is open during hours that are convenient to most people’s schedules.
You’re an applicable patient if…
You work and do not have medical insurance
You live in Sumner County
You live without medical insurance in a qualifying household
You don’t have TennCare
Steps to becoming a Salvus Center patient:
Send in Your Application
Download the new patient application, fill it out,
and send it to Jennifer Wright, Office Manager at jwright@salvuscenter.org
Make an Appointment
Once you receive approval, call us at 615-451-0038 to make your first appointment and receive further details.
At the Appointment
Come to the office on the day of your scheduled appointment 15 minutes early.
Amount due ranges based on scale via cash or check as we do not accept credit cards. If needed, please bring a translator.
Make sure you have:
- A tax return (or fax this beforehand)
- At least 3 pay stubs • Mail (proof of Sumner County residence)
- Download and fill out paperwork (or arrive early and fill it out at the appointment)
